Can be picked up at the Dispatch office at
the police department. All forms need to be returned to the
Chief of Police, Kenneth Campbell. These forms can be mailed
to the above address or can be returned to the police
department in a sealed envelope.

This form can be
obtained at the Santa Fe Police Department Records Office or
Dispatch Office. Once the form is completed, please return it
to the Records Department. The Records Request will be turned
into the Chief of Police in order for him to determine if the
records can be released. Public copies of reports can be
